Oakville School District-School Based Health Center (SBHC)

It's fast & easy for your child to receive health care services through the Oakville School District-School Based Health Center!
Dear Parent or Guardian,
We are happy to inform you that the Oakville School District has a School-Based Health Center (SBHC)!! The SBHC is run by a Department of Health grant managed by the Grays Harbor Public Health Department, The SBHC currently provides services once a week during the academic day, when school is open and in session. Summer hours may be added in the future.
Please know that your child can use the School-Based Health Center and see other doctors as well. Signing the consent does not change your insurance, does not change your private doctor, and does not affect the number of times your child can see their primary doctor.
Cost. At the School-Based Health Center, your child can receive the services listed below at no cost to you, regardless of insurance status. The SBHC is allowed to bill insurance, however there are no co-pays for you, and you do not receive a bill. Please note, you MAY need to pay for any treatment or testing that is completed outside of SBHC (i.e. blood work, lab cultures, x-rays, etc.) and/or for referrals that result in specialty care. If you do not have health insurance, the SBHC's staff can help you enroll in an insurance plan.
Services. Our medical services include primary and preventative health care, diagnosis and treatment of medical conditions, management of chronic conditions, sick visits (i.e. ear infections & strep throat), and referrals to specialty care when necessary. We also provide physicals (annual, sport or camp).
To use this service, please complete the following registration pack in its entirety. SBHC forms can be returned to the school's main administrative office. Please do not return them to your teacher.
